Summer 2019 Soccer Tryouts

I am 18 years of age or older and wish to participate in tryouts for Hawkeye Community College RedTail Dance. I fully understand and acknowledge that physical exertion has inherent risks, dangers, and hazards and my participation in or observation of such classes or activities may result in serious injury or illness or even death. These risks and dangers may be caused by the negligence of the officers, trustees, agents, representatives, students, employees, or volunteers of Hawkeye Community College or its related entities (collectively, "Hawkeye Community College"), the negligence of participants or others, accidents, breaches of contract, or other causes. I understand that one purpose of this document is to bring to my attention the existence of potential dangers associated with participation in tryouts and to aid me in making a voluntary and informed decision as to whether I should participate in such activities. I have had an opportunity to inspect the facilities and equipment or immediately upon entering the facility will inspect such facilities and equipment and have accepted the same as being safe and reasonably suited for the intended purposes. It is my responsibility to learn about and inquire of physicians, attorneys, or other knowledgeable persons about any concerns that I might have regarding the safety of such activities. I am encouraged to have a complete physical examination prior to participating in any tryout for Hawkeye Community College; if I have any health problems or concerns due to injury, illness, age, or a history of heart disease or high blood pressure, respiratory problems, skeletal or muscular injuries or ailments or am significantly above my recommended weight based on a standard weight chart. I may also seek legal counsel before signing this form. I understand these risks and dangers and am voluntarily participating in the Hawkeye Community College RedTail Dance Tryouts. By my signature below, I hereby assume all the risks and dangers and all responsibility for any losses or damages whether caused by myself, or in whole or in part by the negligence or conduct of Hawkeye Community College, or by any other person, and, on behalf of myself, my personal representatives and heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend, and indemnify, to the fullest extent provided by law, Hawkeye Community College from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of consortium or services or otherwise which may arise out of my use or observation of any facilities, services, equipment or participation in the programs or activities. I specifically understand that I am releasing, discharging, and waiving any claims or actions that I may have presently or in the future for the negligent acts or other conduct by Hawkeye Community College. I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND IT, AND FREELY SIGN IT. IT IS MY INTENTION TO FULLY EXEMPT AND RELIEVE HAWKEYE COMMUNITY COLLEGE FROM ANY LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE.

In connection with electronic/printed announcements being produced by Hawkeye Community College, I hereby grant Hawkeye Community College, its successors, assigns, those actin under its permission, or upon its authority permission to use my likeness in a photograph, in any and all of its publications, including website entries. I further hereby consent that Hawkeye Community College may take video or audio and that Hawkeye Community College may use, reproduce and publish the same in any manner and context and with which sounds and dialogue and copy as Hawkeye Community College deems advisable. I understand and agree that these materials will become property of the Hawkeye Community College and will not be returned. I understand and agree the College may edit, alter, copy, exhibit, publish or distribute this photograph, video, or audio for purposes of publicizing Hawkeye Community College or for any other lawful purpose. I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears.
